Job summary

Yale New Haven Health Hospice is seeking a devoted Registered Nurse Case Manager to join the interdisciplinary team. You will provide compassionate, comprehensive care at the patient’s home, develop and update the plan of care in collaboration with physicians and other disciplines, and ensure timely CMS data collection and documentation.

Qualifications

• Nursing diploma/ASN required; BSN preferred.
• Minimum 2 years nursing experience; hospice/home care preferred.
• Connecticut RN license required; CPR certification required; driver license.
• Ability to travel to multiple patient stops daily.

Responsibilities

• Assumes primary responsibility for assigned patients and performs all phases of the nursing process.
• Develops patient- and family-centered plans of care; coordinates with providers and disciplines.
• Educates patients and caregivers about hospice philosophy, disease progression, and care plans.
• Completes EMR documentation within 24 hours with updates on condition and follow-up.
• Oversees LPNs and Home Health Aides; coordinates POC updates per regulatory standards.
• Schedules patients independently according to the plan of care; participates in IDT meetings.
