Job summary

Yale New Haven Health at Home in Milford, CT is seeking a Registered Nurse Case Manager to admit patients for home care, develop plans of care, coordinate with providers, and ensure safe delivery of patient care.

You will collaborate with the multidisciplinary team, educate patients and caregivers, and manage documentation and schedules across multiple home visits. A CT RN license and CPR are required.

Qualifications

• RN with Connecticut license required.
• Minimum two years of nursing experience in an acute care setting.
• Home health experience is desirable but not required.
• CPR certification required and must be obtained within 3 months of hire.
• Must be able to travel to multiple stops daily.
• Excellent clinical, verbal and written skills; able to work independently.

Responsibilities

• Assess and reassess patients and establish patient-specific plans of care.
• Collaborate with providers and other disciplines; communicate changes promptly.
• Assess payment sources and ensure ongoing authorizations as needed.
• Educate patients and caregivers in ongoing medical maintenance; begin discharge planning.
• Complete all documentation within 24 hours and document changes in condition.
• Evaluate patient progress and take prompt action as needed.
• Oversee LPNs and Home Health Aides.
• Schedule patients independently according to the care plan.
• Participate in after-hours, weekend, and holiday coverage as needed.
• May perform other duties as assigned.
