Job summary

Advocate Health Milwaukee seeks a Nurse Case Manager for the Milwaukee South area. This full-time role covers a caseload with late hours (10am-8pm), four days per week, and occasional weekends or on-call nights to support continuity of care within the Milwaukee Metro.

The position requires an Associate’s Degree in Nursing, active RN license, BLS certification and valid driver’s license. The role offers competitive pay, sign-on eligibility, mileage reimbursement, and comprehensive benefits

Qualifications

• Associate's Degree in Nursing.
• Registered Nurse license issued by the state of practice.
• BLS for Healthcare Providers certification.

Responsibilities

• Manage caseloads in specialty areas.
• Conduct initial assessments covering physical, social and environmental data.
• Establish care plans per physician orders and patient needs.
• Provide direct patient care and adjust nursing processes as needed.
• Update and revise care plans; assess patient response.
• Coordinate with family and health team as appropriate.
• Document visit assessments, OASIS, 485 and orders accurately.
• Communicate with physicians to update care plans and ensure continuity.
