Qualifications

• Active Washington RN license required.
• Current CPR certification.
• Valid driver's license and auto insurance.

Responsibilities

• Conduct initial and ongoing patient assessments using AI-enabled charting.
• Develop and update individualized care plans.
• Deliver patient-centered nursing care and education.
• Collaborate with interdisciplinary team members.
• Support patients and families with chronic disease management and wellness strategies.
