Sunrise Home Health Services is seeking a compassionate Registered Nurse (RN) Case Manager for a dedicated Scottsdale territory serving Scottsdale, Paradise Valley, Tempe, and surrounding East Valley communities. Expect $85,000–$100,000 per year with a sign-on bonus for qualifying full-time candidates.

The role emphasizes meaningful one-on-one patient relationships, manageable caseloads, and strong clinical support from experienced managers to ensure high-quality home health care.

Qualificações

• Current Arizona RN license (or Compact license eligible)
• Graduate of an accredited nursing program
• Current CPR Certification
• Valid Arizona driver's license
• Reliable transportation and auto insurance
• Home health experience (preferred)
• OASIS documentation experience (preferred)
• Case management experience (preferred)

Responsabilidades

• Manage an assigned caseload within the Scottsdale territory
• Complete SOC, ROC, Recertification and Discharge assessments
• Perform comprehensive nursing assessments and develop individualized plans of care
• Provide skilled nursing care, medication management, wound care, and patient education
• Monitor patient progress and communicate changes with physicians and the care team
• Coordinate care with Physical/Occupational/Speech Therapists and Medical Social Workers
• Complete timely and accurate documentation in the EMR
• Help reduce hospitalizations while improving patient independence and quality of life
