Job summary

Manito Home Health in Spokane is seeking a full-time Registered Nurse Case Manager to deliver high-quality home health care. The role offers autonomy, one-on-one patient relationships, and a path for professional growth within The Pennant Group.

The RN will conduct comprehensive assessments, develop care plans, and coordinate interdisciplinary care across home settings while maintaining documentation and ensuring safe patient transitions.

Qualifications

• Current Washington RN license (or ability to obtain)
• One year of nursing experience preferred; home-health experience a plus
• Strong assessment, communication, and critical-thinking skills
• Ability to work independently in a field-based role
• Reliable transportation and valid driver’s license

Responsibilities

• Complete comprehensive nursing assessments and develop individualized care plans
• Provide skilled nursing interventions in patient homes
• Manage a balanced caseload with strong organizational efficiency
• Coordinate care with interdisciplinary team members
• Educate patients and caregivers on disease processes, medications, and safety
• Maintain accurate, timely documentation
• Communicate effectively with physicians, families, and internal team members
• Support safe transitions from hospitals, SNFs, and outpatient settings
