Job summary

Professional Case Management is seeking RN Case Managers in Washington to deliver in-home nursing care and comprehensive case management for clients who require ongoing health support. You will assess needs, develop care plans, and coordinate with regional leadership to ensure high-quality home-based services.

Qualifications

  • Graduate of a state approved school of professional registered nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two years nursing experience including one year in home care.
  • One year of supervisory and/or case management experience preferred.
  • BSN preferred.
  • Current CPR certification.

Responsibilities

  • Provide direct care and case management of assigned clients.
  • Plan, implement, and evaluate care provided; participate in care conferences as needed.
  • Coordinate and manage client care with regional director and staff.
  • Assist staffing coordinators with scheduling and staffing within scope of practice.
  • Ensure adherence to nursing delegation guidelines and agency policies.
  • Perform supervisory visits and contribute to performance improvement initiatives.
