Job summary

Sunrise Home Health Services is seeking a compassionate RN Case Manager to serve the Northwest Phoenix Valley, including Sun City, Surprise, Peoria, and surrounding communities. You will combine hands-on nursing with clinical case management to help patients recover and remain independent at home.

The role offers a dedicated territory, manageable caseloads, and robust clinical support from experienced managers to ensure high-quality patient care.

Qualifications

• Current Arizona RN License and have graduated from an accredited nursing program.
• Current CPR Certification; valid driver's license; reliable transportation.
• Strong clinical assessment, communication, and organizational skills; ability to work independently in home-based settings.

Responsibilities

• Manage an assigned caseload within the Northwest Phoenix Valley.
• Complete SOC, ROC, Recertification, and Discharge assessments.
• Develop individualized plans of care and provide skilled nursing care.
• Provide wound care and monitor healing and treatment effectiveness.
• Manage medications and patient education; ensure timely documentation per Medicare and agency requirements.
• Coordinate care with therapists, social workers, physicians, and other professionals.
• Support patient safety and reduce hospitalizations.
