Job summary

Elite Home Health & Hospice in Moscow, Idaho, is seeking a dedicated Registered Nurse (RN) Case Manager to coordinate and deliver high-quality home health and hospice services. Qualifications

Current Idaho RN license in good standing.
Graduate of an accredited nursing program.
One year of nursing experience required.
Home health or hospice experience preferred.
Strong assessment, communication, and organizational skills.
Ability to work independently and manage a caseload.
Current driver's license and reliable transportation.

Responsibilities

Complete comprehensive patient assessments and develop individualized care plans.
Coordinate care with physicians, therapists, social workers, and other healthcare professionals.
Provide skilled nursing care according to physician orders and agency policies.
Monitor patient progress and adjust care plans as needed.
Educate patients and families about disease management, medications, and plan of care.
Maintain accurate and timely clinical documentation.
Participate in interdisciplinary team meetings and case conferences.
Ensure compliance with federal, state, and agency regulations.
