Job summary

Angels Care Home Health in Virginia is seeking an experienced Registered Nurse Case Manager to coordinate all aspects of patient home care. You will perform admission visits, assess eligibility, and develop the plan of care in collaboration with the inter-disciplinary team.

Requirements include graduation from an accredited nursing program (diploma/ADN/BSN), an active RN license, a valid driver’s license, CPR certification, and at least one year of RN experience.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Valid driver’s license and reliable transportation.
• One year of RN experience; two years preferred.
• CPR certification and Hepatitis consent/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with medical disciplines as needed to provide professional care.
• Perform admission visit and determine eligibility for home care services.
• Develop and follow the plan of care with the interdisciplinary team.
