Job summary

Angels Care Home Health in the United States is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of a patient’s home care. You will perform admission visits, assess eligibility for services, and develop the plan of care in collaboration with the care team.

Qualified candidates are graduates of accredited nursing programs, hold a current RN license, possess a valid driver’s license, CPR certification, Hepatitis consent/declination, and reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state RN license.
• Valid driver's license with reliable transportation.
• Proof of CPR and Hepatitis consent/declination.

Responsibilities

• Coordinate all aspects of the patient’s home care and interact with the medical team as needed.
• Perform admission visit to the patient in the home and determine eligibility for home care services.
• Develop and follow the plan of care in collaboration with the care team.
