Angels Care Home Health in Davenport, IA is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate home-based patient care. You will assess patient needs, develop and implement the plan of care, and collaborate with interdisciplinary teams to ensure high-quality outcomes.

The ideal candidate holds an accredited nursing degree, an active RN license in Iowa, a valid driver's license, and at least 1 year of RN experience (2 preferred).

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsabilités

• The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform admission visits to determine eligibility for home care services and develop the plan of care.
