Angels Care Home Health in Bedford, IA is seeking an experienced RN Case Manager to coordinate patient home care and collaborate with medical teams to deliver high-quality nursing services in the patient’s home.

Qualifications

• Graduate of an accredited diploma, associate or baccalaureate school of nursing.
• Current state RN license and a valid driver’s license.
• One year RN experience; two years preferred in home health.
• Proof of CPR and Hepatitis consent/declination; reliable transportation with insurance.

Responsabilités

• Coordinate all aspects of the patient’s home care as the RN Case Manager.
• Interact with other medical disciplines as needed and provide professional care.
• Perform admission visits, determine eligibility for home care, and develop the plan of care.
