Angels Care Home Health in Denison, IA is seeking a Registered Nurse (RN) Case Manager to coordinate comprehensive in-home nursing care. You will interact with multiple medical disciplines and deliver professional patient care, starting with the admission visit and assessing eligibility for home care services.

The position emphasizes developing and following a personalized plan of care, ensuring quality outcomes for patients in their homes.

Formación

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.

Responsabilidades

• Coordinate all aspects of the patient’s home care. Interact with other medical disciplines as needed and provide professional care.
• Perform the admission visit to the patient in the home and determine eligibility for home care services.
• Develop the plan of care to be followed.
