Job summary

Iowa Home Care seeks an experienced Registered Nurse Case Manager to coordinate comprehensive home health services across Polk County. You will assess patients, create individualized plans of care, and lead a multidisciplinary team to ensure seamless, safe, and effective care in the home setting.

The role emphasizes clinical autonomy, organization, and strong communication to keep patients and families informed and engaged in achieving health goals.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills; problem solving; basic math.
• Licensed driver with reliable vehicle and good insurance.

Responsibilities

• Complete start-of-care and ongoing patient assessments as required.
• Develop individualized plans of care based on patient needs and physician orders.
• Provide skilled nursing services including medications and disease management.
• Monitor patient progress and adjust plans as needed.
• Coordinate nursing, therapy, aide, and other services involved in care.
• Educate patients and caregivers on medications, safety, and home management.
• Document thoroughly and timely in the medical record.
• Advocate for patients and identify resources to support health and independence.
