Job summary

Iowa Home Care is seeking an experienced Registered Nurse to serve as a Case Manager in Ottumwa, IA. You will oversee patient care plans, coordinate with an interdisciplinary team, and deliver high-quality home health services in patients’ homes.

The role includes assessments, direct nursing care, education for patients and families, and supervision of home health aides. Strong clinical skills and valid RN licensure are required.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience in an institutional setting.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills; problem solving; basic math skills.

Responsibilities

• Assess patients and develop individualized home care plans.
• Administer medications and provide skilled nursing care as prescribed.
• Coordinate care with physicians and other health providers to optimize outcomes.
• Maintain timely clinical notes and communicate changes to the physician.
• Educate patients and families about care plans and health management.
• Supervise and evaluate home health aides to ensure quality care.
