Job summary

Iowa Home Care is seeking an experienced Registered Nurse to join our home care team as a Case Manager. You will assess patients in their homes, develop individualized care plans, and coordinate with an interdisciplinary team to ensure high-quality outcomes.

You will provide direct nursing care, administer medications, and guide patients and families in health management. You will supervise home health aides bi-weekly to ensure care standards are met.

Qualifications

• Graduate of an accredited nursing program.
• 1–2 years of recent acute care experience in an institutional setting.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills, problem solving skills; basic math skills.
• Licensed driver with automobile in good working order and insured as required.

Responsibilities

• Assess patients and plan individualized home care.
• Provide direct patient care including medications and nursing procedures.
• Coordinate care across services with physicians and community providers.
• Document with accurate clinical notes and update primary physician as needed.
