Iowa Home Care is seeking a Registered Nurse to serve as a Home Health Case Manager, delivering patient-centered care directly in clients' homes. You will assess needs, develop care plans, and collaborate with an interdisciplinary team to ensure high standards of health and wellness.

Responsibilities include patient assessments, direct nursing care, care coordination, documentation, patient education, and supervising health aides.

Qualificações

• Graduate of an accredited school of nursing.
• Current RN licensure and CPR certification.
• Home health or case management experience is a plus.

Responsabilidades

• Assess patients and plan care, updating individualized care plans.
• Provide direct nursing care and administer prescribed treatments.
• Coordinate care across services with physicians and providers.
• Maintain timely clinical notes and inform the physician of changes.
