Job summary

Iowa Home Care Llc is seeking a Registered Nurse Case Manager to oversee patients across Marion County. In this role you’ll combine clinical nursing skills with care coordination, patient education, and advocacy to ensure services are delivered at the right time and place.

You’ll build relationships with patients and families, work closely with physicians, therapists, and home health aides, and help coordinate a comprehensive plan of care in the patient’s home.

Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of recent acute care experience in an institutional setting.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills.
• Licensed driver with reliable, insured vehicle.

Responsibilities

• Perform comprehensive nursing assessments at admission and throughout the care episode.
• Identify patient and family needs and establish goals and interventions.
• Create and update individualized plans of care and provide skilled nursing treatments.
• Coordinate nursing, therapy, aide, and other services involved in the patient’s care.
• Educate patients and families on medications, disease management, safety, and self-care.
