Qualifications

• Graduate of an accredited school of nursing.
• 1–2 years of acute care experience in an institutional setting.
• Current RN licensure and CPR certification.
• Licensed driver with automobile in good working order and insured.

Responsibilities

• Assess patients and plan individualized home care using comprehensive care plans.
• Administer medications and provide skilled nursing care as prescribed.
• Coordinate care across services and collaborate with physicians and health providers.
• Document clinical notes accurately and update the physician at required intervals.
• Educate patients and families about health conditions and home care plans.
