Job summary

Iowa Home Care in West Des Moines, IA is seeking a Registered Nurse Case Manager to coordinate patient care in clients' homes, develop individualized care plans, and collaborate with an interdisciplinary team.

You will perform patient assessments, administer treatments as prescribed, educate families, supervise home health aides, and ensure high-quality, compassionate care within the home setting.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure and CPR certification.
• Strong observation, communication, and problem-solving skills.
• Licensed driver with reliable vehicle.

Responsibilities

• Patient Assessment & Care Planning: assess patients and create individualized home care plans.
• Direct Patient Care: administer meds and skilled nursing care per state act.
• Care Coordination: coordinate across services with physicians and community providers.
• Documentation & Communication: maintain clinical notes and update physicians every 60 days.
• Patient & Family Education: provide health instructions and involve families in planning.
• Supervision of Support Staff: supervise and evaluate home health aides bi-weekly.
