Job summary

SunnyBrook Home Care in Ottumwa, IA is seeking a Registered Nurse for a full-time Case Management position in the home health setting. The role emphasizes one-on-one patient care, care coordination, and adherence to Medicare/Medicaid guidelines.

New management promises supportive leadership, on-the-job training, and flexible scheduling with a Monday–Friday core hours and on-call rotation. NEW grads welcome; experience in home health preferred but not required.

Qualifications

• Active Iowa RN Licensure.
• CPR Certification.
• Must have a valid driver's license, insurance and reliable transportation.
• Prior home care experience is preferred but will train the right candidate.
• Experience with OASIS, Medicare/Medicaid regulations, and EMR (Wellsky/Kinnser) software preferred but will train the right candidate.
• Ability to work independently with good time management/organizational skills as well as part of a close supportive team.
