SunnyBrook Home Care is seeking a Registered Nurse for a full-time Case Management position in the Washington, Iowa area. Home Health experience is preferred but not required; on-the-job training provided for qualified candidates. NEW GRADS WELCOME!

Qualifications

• Active Iowa RN Licensure.
• CPR Certification.
• Must have a valid driver's license, insurance and reliable transportation.
• Prior home care experience is preferred but will train the right candidate.
• Experience with OASIS, Medicare/Medicaid regulations, and EMR (WellSky/Kinnser) software preferred but will train the right candidate.
• Ability to work independently with good time management/organizational skills as well as part of a close supportive team.

Responsibilities

• Provide one-on-one case management in patients' homes.
• Coordinate care with healthcare providers and family members.
• Participate in on-the-job training and orientation with clinical leadership.
