Job summary

To The Rescue is seeking a Registered Nurse (RN) – Home Health Case Manager to provide skilled nursing and care coordination for adult patients in their homes in Cedar Rapids, IA. This full-time role offers meaningful one-on-one care and professional autonomy.

You’ll manage patient care from admission to discharge, collaborating with physicians, therapists, patients, and families to help individuals remain safe and independent at home. A $15,000 sign-on bonus is offered for full-time nurses.

Qualifications

• Active RN license in Iowa (or eligible).
• Graduate of an accredited nursing program (ADN/BSN).
• Minimum of 1 year nursing experience; home health preferred.

Responsibilities

• Provide skilled nursing visits per physician orders and care plans.
• Complete comprehensive patient assessments and updates.
• Coordinate services with care team and families.
• Educate patients to support safety and independence at home.
• Document care accurately per standards and regulations.
