Job summary

Iowa Home Care is seeking a Registered Nurse Case Manager to deliver patient-centered care in clients' homes across Indianola, IA. You will assess needs, develop individualized care plans, and coordinate with a multidisciplinary team to ensure high-quality outcomes.

As an RN, you will perform direct patient care, guidance to families, maintain documentation, and supervise home health aides while complying with state nurse practice standards and company policies.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN licensure and CPR certification.
• 1–2 years of acute care experience in a clinical setting.

Responsibilities

• Patient Assessment & Care Planning: assess patients and families to determine home care needs and create individualized care plans.
• Direct Patient Care: administer medications and nursing care per state Act and plan.
• Care Coordination: coordinate across services with physicians and providers to optimize plans.
• Documentation & Communication: maintain timely clinical notes and update physicians as needed.
• Patient & Family Education: provide tailored health instructions and involve families in planning.
• Supervision of Support Staff: supervise home health aides bi-weekly to ensure care quality.
