Job summary

Iowa Home Care is seeking a Registered Nurse Case Manager to oversee and coordinate patient care in clients' homes. You will assess patients, develop individualized care plans, and work with an interdisciplinary team to maintain high standards of health and wellness.

The role requires an active RN license and CPR, with 1–2 years of acute care experience. A valid driver's license and reliable vehicle are expected. This is a full-time, day-shift position in West Des Moines, IA.

Qualifications

• Graduate of an accredited nursing program; 1–2 years of acute care experience.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills; basic math; nursing skills per competency.
• Licensed driver with automobile in good working order and insured.

Responsibilities

• Patient assessment and care planning: assess patients and families to determine home care needs; develop individualized care plans.
• Direct patient care: administer medications and treatments; provide skilled nursing care per state guidelines.
• Care coordination: coordinate across services; collaborate with physicians and providers to optimize outcomes.
• Documentation and communication: maintain clinical notes; notify physician of changes and get orders as needed.
• Patient and family education: provide health instructions and involve families in planning.
• Supervision of support staff: supervise and evaluate home health aides bi-weekly.
