Job summary

Iowa Home Care is seeking a Registered Nurse Case Manager to provide patient-centered home health services in Ottumwa, IA. You will develop individualized care plans and collaborate with an interdisciplinary team to ensure high-quality health outcomes.

The role requires RN licensure, CPR certification, and 1–2 years of acute care experience. You will lead care coordination, document care, and educate patients and families, with flexible scheduling and strong support from the organization.

Qualifications

• Graduate of an accredited school of nursing.
• One to two years of recent acute care experience in an institutional setting.
• Current RN licensure and CPR certification.
• Excellent observation, verbal and written communication skills; problem solving and basic math skills.
• Licensed driver with a vehicle in good working order and insured per organizational requirements.

Responsibilities

• Assess patients and plan individualized home care.
• Administer medications and provide skilled nursing care.
• Coordinate care with physicians and health providers.
• Maintain accurate clinical notes and report changes to physicians.
• Educate patients and families about health needs and care plans.
• Supervise and evaluate home health aides bi-weekly.
