Qualifications
- Graduate of an accredited school of nursing.
- 1–2 years of recent acute care in an institutional setting.
- Current RN licensure and CPR certification.
- Licensed driver with automobile in good working order.
Responsibilities
- Assess patients and develop individualized home care plans.
- Administer medications and skilled nursing care as prescribed.
- Coordinate care with physicians and community health providers.
- Document clinical notes and communicate status changes to physicians.
- Supervise home health aides bi-weekly to ensure quality care.
