Job summary

Alternate Solutions Home Health is seeking a Registered Nurse Case Manager to lead patient assessments, plan of care and coordination of multidisciplinary teams in the Burke, Fairfax, Springfield and surrounding areas. The RNCM will educate patients and families, document accurately, and ensure compliance with state regulations.

A valid RN license, 1+ year acute care experience, and willingness to travel are required.

Qualifications

• RN with current license in the state of employment.
• Minimum of one year of RN experience in an acute care setting.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Compassionate and caring demeanor while working with patients.
• Knowledgeable on nursing best practices and sound nursing judgments.
• Detail-oriented with disciplined work ethic and teamwork.

Responsibilities

• Oversees the overall care of the patient from beginning to end of episode.
• Updates all doctor orders in patient chart accordingly.
• Notifies doctor of significant changes in patient’s condition.
• Manages multi-disciplinary care as applicable.
• Provides skilled nursing care in a patient’s home as ordered by the physician.
• Observes and monitors patient conditions.
• Documents all clinical care and education per agency protocol.
