Job summary

Alternate Solutions Home Health, LLC is seeking a Registered Nurse Case Manager to lead assessment, care planning, and coordination of multi-disciplinary care in patients' homes within the Burke, Fairfax, Springfield and surrounding VA areas.

The RNCM will educate patients and families, document accurately, and ensure compliance with state and Medicare guidelines while providing high-quality, cost-effective care. A sign-on bonus is available.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum of one year of experience as an RN in an acute care setting.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Ability to remain calm, patient, and accommodating.
• Compassionate and caring while working with patients.
• Knowledgeable on nursing best practices.
• Ability to make appropriate nursing judgments.
• Identify situations and handle with best possible solution.
• Detail-oriented and observant.

Responsibilities

• Oversee overall care of the patient from start to end of episode.
• Update doctor orders in patient chart as needed.
• Notify doctor of significant changes in patient condition.
• Manage multi-disciplinary care as applicable.
• Provide skilled nursing care in patient’s home as ordered by attending physician.
• Observe and monitor patient conditions.
• Perform OASIS assessments to develop an individualized plan of care and adjust as needs change.
• Administer medications as prescribed by the physician.
• Help decrease hospitalizations by front loading visits for high-risk patients.
• Promote continuity of care with admissions, transfers and discharges.
• Counsel patient and family on disease processes and management.
• Incorporate patient and family in development of plan of care.
• Help decrease hospitalizations by teaching preventative measures and decisions.
• Listens to patient and family members.
• Oversees and supervises total care of patient by nurse aides and LPNs.
• Evaluates treatments and condition per plan of care.
• Determine if level of care meets patient needs.
• Communicates with LPNs and nurse aides on supervisory visit results.
• Completes clinical documentation per agency protocol and Medicare guidelines.
• Documents all aspects of treatment, assessments, and patient education.
• Maintains active RN license.
• Communicates with scheduler about changes outside normal.
• Attends on-call requirements and case conferences.
• Participates in trainings and mandatory meetings.
