Job summary

Alternate Solutions Health Network seeks a Registered Nurse Case Manager to lead patient assessment, plan of care, and coordination across disciplines under physician direction. The RN educates patients and families, ensures safety, and maintains compliant documentation.

Home-based care in the Arlington, Alexandria, and surrounding areas is core to this role. Responsibilities include coordinating care, updating orders, monitoring changes, administering medications, and reducing hospitalizations

Responsibilities

• Oversees the overall care of the patient from beginning to end of episode.
• Updates doctor orders in patient chart.
• Notifies doctor of significant changes in patient condition.
• Manages multi-disciplinary care as applicable.
• Provides skilled nursing care in a patient’s home as ordered by the physician.
• Observes and monitors patient conditions.
• Performs OASIS assessments to develop and adjust the plan of care.
• Administers medication as prescribed by the physician.
• Educates patients and families and ensures safety.
• Documents all aspects of treatment and education in a timely manner.
• Maintains active RN license and compliance with regulations.
