Job summary

Alternate Solutions Home Health, LLC in Fairfax, VA is seeking a Registered Nurse Case Manager (RNCM) who leads assessment, care planning, and coordination across disciplines to deliver high-quality, cost-effective home health services.

The RNCM educates patients and families, ensures documentation accuracy, maintains compliance with Medicare guidelines, and supervises licensed staff to meet each patient’s plan of care in collaboration with the physician.

Qualifications

• Minimum of one year of RN experience in an acute care setting.
• Home care experience is desired.
• Ability to communicate effectively with patients, families, physicians and co-workers.
• Valid driver’s license and auto insurance with your name as a listed driver.

Responsibilities

• Oversees the overall care of the patient from beginning to end of episode.
• Updates all doctor orders in patient chart accordingly.
• Notifies doctor of any significant changes of the patient’s condition.
• Manages multi-disciplinary care as applicable.
• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Observes and monitors patient conditions.
• Performs OASIS assessments to develop an individualized plan of care.
• Completes and submits all required documentation within company requirements.
