Job summary

Traditional Home Health Care in Scranton, PA is seeking a Registered Nurse Case Manager to join our home health team. The role focuses on creating personalized care plans and delivering one-on-one patient care in patients’ homes.

You will assess patients, administer medications, and coordinate with physicians, therapists, and families. A PA RN license and BLS are required, with driving responsibilities and strong documentation skills emphasized.

Qualifications

• Active RN license in Pennsylvania.
• BLS certification.
• Valid driver’s license with reliable transportation.
• Strong assessment and documentation skills.
• Ability to work independently and manage time.
• Excellent communication and interpersonal skills.
• Acute care and/or home health experience preferred.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
• Provide hands-on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care.
• Educate patients and families on treatment plans and self-care techniques.
• Inform patients of additional services and resources as needed.
