Job summary

Interim HealthCare of Scranton is seeking an experienced Registered Nurse (RN) Case Manager for home health in Scranton, PA and surrounding areas. You will manage a home health caseload, perform RN assessments, develop and update care plans, and coordinate care with physicians and the healthcare team.

Applicants must hold an active Pennsylvania RN license, have at least 1 year of home health RN experience, and CPR certification. A valid driver’s license and reliable transportation are required.
