Job summary

Interim HealthCare of Scranton is seeking a Registered Nurse (RN) Case Manager for home health in Scranton, PA and surrounding areas. You will manage your own caseload, perform RN assessments, develop care plans, coordinate with physicians, complete documentation, and educate patients and families.

The role offers a Monday–Friday schedule, on-call rotations, and comprehensive benefits. PA RN license and CPR are required, with mileage reimbursement and weekly pay.

Qualifications

• Active Pennsylvania RN license required.
• Minimum 1 year home health RN experience.
• CPR certification required.
• Valid driver’s license and reliable transportation required.

Registered Nurse RN Case Manager Home Health Lackawanna County Interim HealthCare of Scranton $38–$40/hour + weekly pay

Immediate opening for a Registered Nurse RN Case Manager Home Health in Scranton, PAand surrounding areas. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community.

PAY AND BENEFITS

• $38–$40 per hour

SCHEDULE

• Monday through Friday 8:30 AM to 5:00 PM
• On-call, weekend, and holiday rotation
