Job summary

Interim HealthCare of Scranton is seeking a Registered Nurse RN Case Manager in Scranton, PA to manage a home health caseload, perform assessments, develop and update care plans, and coordinate with physicians.

The role requires an active PA RN license, at least 1 year of home health experience, CPR, and a valid driver’s license. Competitive hourly pay, weekly direct deposit, and benefits are provided.

Qualifications

• Case Manager experience preferred.
• CPR certification.
• Valid driver’s license and reliable transportation.

Immediate opening for a Registered Nurse RN Case Manager Home Health in Scranton, PAand surrounding areas. Manage your own caseload, provide one-on-one patient care, and avoid hospital burnout while working in the community.

SCHEDULE

• Monday through Friday 8:30 AM to 5:00 PM
• On-call, weekend, and holiday rotation

• Manage a home health caseload in Scranton, PA and surrounding areas
