Job summary

Traditional Home Health Care is seeking a Registered Nurse Case Manager to coordinate patient care in the Waymart, PA area. The role involves assessing patients, developing care plans with clinicians and families, and delivering hands-on nursing care in the home setting.

The ideal candidate holds an active PA RN license, BLS, and can manage documentation and time independently. Service area includes Waymart and surrounding regions; full-time daytime schedule.

Qualifications

• Active RN License in Pennsylvania.
• BLS Certification required.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Responsibilities

• Assess patients and develop personalized care plans with physicians, nursing supervisors, patients, and families.
• Provide hands‑on care, administer medications, and evaluate patient progress.
• Maintain and submit timely documentation, including OASIS assessments.
• Collaborate with therapists, caregivers, and healthcare professionals to ensure comprehensive care.
• Educate patients and families on treatment plans and self‑care techniques.
• Inform patients of additional services and resources as needed.
