Job summary

Traditional Home Health Care in Waymart, PA is hiring a Registered Nurse Case Manager to assess patients, create care plans, and coordinate in-home nursing services. The role offers full-time hours Mon-Fri with a focus on independent, patient-centered care and strong team support.

The ideal candidate holds an active PA RN license, BLS, and a valid driver's license. Prior home health experience is preferred and the organization emphasizes quality patient outcomes and supportive collaboration.

Qualifications

• Active RN license in Pennsylvania
• BLS certification
• Valid driver’s license with reliable transportation
• Strong assessment and documentation skills
• Ability to work independently and manage time effectively
• Excellent communication and interpersonal skills
• Acute care and/or home health experience preferred

Responsibilities

• Assess patients and develop personalized care plans with physicians and families.
• Provide hands-on nursing care and administer medications.
• Maintain timely documentation including OASIS assessments.
• Collaborate with therapists and caregivers for comprehensive care.
• Educate patients and families on treatment plans and self-care.
• Identify and connect patients with additional services and resources.
