Job summary

Seva Home Health in Madera, CA is seeking an RN Case Manager to cover Madera and Fresno Counties. This full-time role focuses on developing and implementing patient care plans in home health settings with supportive team collaboration.

Qualified candidates will have 1+ year RN experience, CPR/BLS, and a willingness to drive to patient homes. Training and a compassionate, community-focused environment are provided.

Qualifications

Minimum of 1 year of RN experience with a valid license
CPR/BLS certification is required
Reliable transportation for home visits
Experience with Kinnser EMR strongly preferred
Comfortable driving to patient homes for visits
Prior experience in Palliative/Home Health/Hospice is preferred

Responsibilities

Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient and team.
Establish patient-specific goals and provide nursing services.
Oversee and deliver quality, patient-centered care in the home setting.
Communicate patient status and needs with the care team.
Consult with patient, family, and team on safety and discharge planning.
Document accurately and timely per the plan of care
