Seva Home Health in Madera, CA is seeking an RN Case Manager to cover Madera and Fresno Counties. This full-time role focuses on developing and implementing patient care plans in home health settings with supportive team collaboration.

Qualified candidates will have 1+ year RN experience, CPR/BLS, and a willingness to drive to patient homes. Training and a compassionate, community-focused environment are provided.

Qualifications

• Minimum of 1 year of RN experience with a valid license
• CPR/BLS certification is required
• Reliable transportation for home visits
• Experience with Kinnser EMR strongly preferred
• Comfortable driving to patient homes for visits
• Prior experience in Palliative/Home Health/Hospice is preferred

Responsibilities

• Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient and team.
• Establish patient-specific goals and provide nursing services.
• Oversee and deliver quality, patient-centered care in the home setting.
• Communicate patient status and needs with the care team.
• Consult with patient, family, and team on safety and discharge planning.
• Document accurately and timely per the plan of care

Requirements

• Minimum of 1 year of experience as a Registered Nurse with a valid license required


• Current CPR/BLS Certification


• Reliable transportation required


• Experience with Kinnser EMR strongly preferred


• Comfortable driving to patient homes for patient visits


• Prior experience with Palliative Care preferred


• Prior experience in Home Health, Hospice, Infusion, or Home Care preferred

Responsibilities

• Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.


• Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s home or living facility.


• Communicate necessary revisions to patient status and/or needs with the entire care team


• Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.


• Provide complete, accurate and timely documentation consistent with the plan of care
