• Establish patient-specific goals and deliver quality care.
• Communicate necessary revisions to patient status with the entire care team.
• Provide complete and accurate documentation consistent with the plan of care.

Skills

Experience as a Registered Nurse

CPR/BLS Certification

Experience with Kinnser EMR

Palliative Care experience

Home Health, Hospice, Infusion, or Home Care experience
