Job summary

Baywood Home Care is seeking a compassionate RN Case Manager to coordinate care for patients in their homes. This part-time, in-person role offers a flexible schedule up to 20 hours per week and competitive pay based on experience.

You will assess patients, develop care plans, coordinate with physicians and caregivers, educate families, and ensure accurate documentation. Minnesota RN license and home health or case management experience are required.

Qualifications

• Current Minnesota RN license required
• Home health, case management, or transitional care experience required
• Strong assessment, communication, and care coordination skills
• At least 21 years old

Responsibilities

• Conduct patient assessments and develop individualized care plans
• Coordinate services with physicians, therapists, caregivers, and community resources
• Monitor patient progress and adjust care plans as needed
• Educate patients and families on medications, treatment plans, and wellness strategies
• Maintain accurate clinical documentation and regulatory compliance
