Job summary

Amedisys seeks a Registered Nurse Case Manager to cover the counties of Russell, Lee and parts of Macon in Alabama. This full-time role operates Monday through Friday, 8am–5pm, and focuses on comprehensive home-based care and collaboration with the care team to promote patient healing.

Qualifications

• 1+ year of clinical RN experience required.
• Active RN license in the state you work.
• CPR certification required.

Responsibilities

• Performs patient assessments and develops plans of care with the team.
• Makes referrals as needed and documents rationale.
• Supervises LPNs and HHAs and provides staff education.
• Completes timely and accurate documentation and communicates progress.
