Job summary

Amedisys is seeking a registered nurse case manager to cover the counties of Russell, Lee, and parts of Macon. This full-time role operates Monday through Friday, 8am–5pm, with responsibilities including patient assessments, care planning, and coordinating referrals with the care team.

You will supervise LPNs and HHAs, complete timely documentation, and communicate patient progress to the clinical manager. CPR-certified, RN licensed, and a valid driver's license are required.

Qualifications

• One (1+) year of clinical experience as a Registered Nurse (RN).
• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Conduct referrals to other disciplines as indicated by patient needs and document rationale.
• Promote patient health through teaching and rehabilitative measures; teach self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation timely and accurately and communicate patient progress to the clinical manager.
• Plan and provide staff education and participate in on-call responsibilities.
• Engage in clinical development and continuing education programs.
