Job summary

Constellation Health Services is seeking a Registered Nurse Case Manager to join our field-based team in Greenwich, CT, delivering home care. You will perform head-to-toe assessments, oversee medications, develop the Plan of Care with the care team, use OASIS for admissions and discharges, educate patients and caregivers, and document on an iPad.

Weekend availability and travel are required; CT RN license and home health experience preferred.

Qualifications

• Registered Nurse licensed in the state of employment (CT), graduate of an accredited RN program.
• 1–2 years RN experience; home care experience preferred.
• OASIS experience strongly desired.
• Experience managing catheters and wound care is a plus.
• Reliable transportation and willingness to commute for in-home care.
• Some weekend days and holiday commitments as needed.
• Basic knowledge of iOS operating system.

Responsibilities

• Provide skilled nursing and head-to-toe assessments for adults and geriatrics.
• Visit assigned caseload with admissions, re-certification, and discharges.
• Develop Plan of Care with all health care professionals involved.
• Be knowledgeable in using the current OASIS for patient admissions and transitions.
• Educate patients and caregivers on disease processes and treatment plans.
• Report to the Clinical Manager daily and document via iPad within required timeframes.
