Job summary

Western Connecticut Home Care is seeking an RN to join a home health team operating under the Northwell Health Network. You will assess needs, plan and implement care, and coordinate with physicians, patients, and families in the home setting.

The role supports a 40-hour work week with day shift and weekend rotation, offering benefits and orientation. CT RN licensure and clinical experience are required, with opportunities for professional growth.

Qualifications

• RN diploma and CT RN license are required.
• Strong research skills and clear interpersonal communication are expected.
• Knowledge of systems integration is a plus.

Responsibilities

• Assess patients' physical, emotional, socio-economic, and environmental needs.
• Provide physician-ordered services per plan of care and coordinate with care team.
• Develop and evaluate the plan of care with patient, family, and physicians.
• Coordinate services and counsel patient and family.
• Evaluate progress and take action as patient condition changes.
• Participate in quality assurance and in-service training.
• Identify additional home health services for patients and refer as needed.
• Supervise home health aides and report evaluations.
• Fulfill compliance responsibilities and prepare clinical notes.
