Job summary

Constellation Home Care is seeking a compassionate Registered Nurse (RN) Case Manager for a field-based role covering an assigned territory. You will perform head-to-toe assessments, oversee medications, and provide home health services in patients’ homes, assisted living, and skilled nursing facilities.

You will develop the Plan of Care with clinicians, utilize OASIS for admissions and discharges, educate patients and caregivers, and document on an iPad. Some weekends and travel are required.

Qualifications

• Registered Nurse licensed in the state of employment and graduate of an accredited RN program.
• At least 1–2 years of RN experience; home care preferred.
• OASIS experience is strongly desired.
• Experience with PleurX/Foley Catheters, Wound Vac, Ostomy care is a plus.
• Reliable transportation and willingness to commute for in-home care.
• Some weekend days required; summer and winter holidays as needed.
• Basic knowledge of iOS operating system.

Responsibilities

• Provide skilled nursing and head-to-toe assessments for adults and geriatric patients.
• Visit patients to deliver home health services, including admissions, re-certifications and discharges.
• Develop the Plan of Care with the care team.
• Utilize current OASIS assessments for admissions, re-certifications, transfers and discharges.
• Educate patients and caregivers on disease processes and treatment plans.
• Report to Clinical Manager daily and document using provided iPad within required time frames.
