Job summary

Honor Health Network is seeking an experienced RN Home Health Case Manager to serve Stratford, CT and surrounding communities. The role offers part-time or full-time options, with compensation between $98,000 and $110,000 per year plus mileage, and a strong emphasis on work-life balance.

You will conduct home visits, develop individualized care plans, coordinate with the care team, and educate patients and families on medications and disease management while maintaining thorough documentation.

Qualifications

• Current, unrestricted RN license in the State of Connecticut.
• Previous home health experience strongly preferred.
• Excellent communication and organizational abilities.
• Ability to work independently while collaborating with a team.

Responsibilities

• Conduct comprehensive nursing assessments during home visits.
• Develop, implement, and update individualized patient care plans.
• Provide skilled nursing care in accordance with physician orders and applicable regulations.
• Monitor patient conditions and identify changes requiring intervention.
• Coordinate care with physicians, therapists, aides, social workers, and other members of the healthcare team.
• Educate patients and caregivers regarding medications, disease management, treatment plans, and preventative care.
• Complete accurate and timely clinical documentation.
• Communicate effectively with patients, families, physicians, and the interdisciplinary team.
• Manage an assigned caseload while maintaining high standards of patient care.
• Help patients and families navigate their healthcare needs and achieve their goals.
