Job summary

Healthcare United is seeking a Registered Nurse/Case Manager for at-home services in the client's home. Hourly rate depends on experience. Positions in Stratford, Hamden, Norwalk, and Middletown offer Full-Time/Part-Time with flexible hours and options to choose 4/40 or 5 days.

You will case manage 10 patients per month and complete at least 5 assessments. Benefits include generous sign-on bonus for full-time, medical, dental, vision, life/disability, and 401k after 6 months, plus paid vacation.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Current license as a Registered Nurse in Connecticut.
• Ability to assess and respond to needs of patients and families in varied settings.
• Ability to make meaningful observations and write comprehensive accurate reports.

Responsibilities

• Case manage 10 patients and complete at least 5 assessments per month.
• Provide at-home nursing care and assessments to clients in Stratford, Hamden, Norwalk and Middletown.
