Job summary

Angels Care Home Health in Lincoln, Nebraska is seeking a Registered Nurse Case Manager to coordinate comprehensive home care. You will perform admission visits, assess eligibility, and develop individualized care plans while collaborating with multidisciplinary teams.

The role requires an RN license, a valid driver’s license, CPR certification, and reliable transportation. Prior home health experience is preferred; care excellence and patient advocacy are core to our values.

Qualifications

• Graduate of an accredited diploma, ASN or BSN program.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year RN experience; two years preferred.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with auto liability insurance.

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Interact with other medical disciplines as needed.
• Perform admission visit to the home and determine eligibility.
• Develop and follow the plan of care for each patient.
